Social Security form SSA 3373-BK

Hi Everyone,

Has anyone in the group applid for and received a favorable Social

Security determinatation? If so, please could you help me.

A couple of weeks ago I received, from Social Security, Form SSA-454-

BK.

I completed and returned. Now, yesterday, I received ANOTHER form,

from Social Security. This form is SSA 3373-BK. This has me very

distraught and depressed and mad.

To get my first disability approval took three (3) years and an

attorney.

The thing that has me stumped is the section of " DESCRIBE WHAT YOU DO

FROM THE TIME YOU WAKE UP UNTIL GOING TO BED. " Which I guess I

didn't answer very well on the first form.

Has anyone in this group had to fill these two (2) forms out? If so,

could you please help me.

Which I mean, could you send me a sample of what you sent to help

me. I am a nervous wreck over this whole thing. I want to make sure

I put in the right things

Any help at all would be greatly appreciated.
