Debbie I don't think I can help u, but know I have a ?? or two. I just

filed for Disability, and now I am discouraged you say it took u 3 yrs? I know

most of the time everyone gets denied the first time. I also know each State is

a bit different, so I don't think I can help but maybe take some of your anxiety

away, and maybe someone else here lives in your state and can answer your

concern. If you are already on disability, is this just like a assesment type

form they do every so often, or have u applied for a 2nd disability? What

answers did u give the first time on your days event? I think they are looking

to see if you have any improvement with your disability. If you are unsure how

to anwer, can u call the disability advocate in your state, and ask what that ?

is about. Usually they are realy good on helping they did me. Don't worry

about it.

I completed and returned. Now, yesterday, I received ANOTHER form,

from Social Security. This form is SSA 3373-BK. This has me very

distraught and depressed and mad.

To get my first disability approval took three (3) years and an

attorney.

The thing that has me stumped is the section of " DESCRIBE WHAT YOU DO

FROM THE TIME YOU WAKE UP UNTIL GOING TO BED. " Which I guess I

didn't answer very well on the first form.

Has anyone in this group had to fill these two (2) forms out? If so,

could you please help me.

Which I mean, could you send me a sample of what you sent to help

me. I am a nervous wreck over this whole thing. I want to make sure

I put in the right things

Any help at all would be greatly appreciated.
