I retired on a medical disability from the federal government.

question

1.Fully describe your disease? Chronic intractable pain caused by multiple

diseases of the spine, including DDD, stenosis, scoliosis, spondylosis,

osteoarthritis, osteoporisis.

2.Describe how your disease effects your daily living

and interferes or restrictions of your activties

imposed by this disease.

I am unable to do most daily activities including difficulties in personal

hygiene, walking, standing, sitting. Unable to do any housework. Extreme

difficulty in driving due to my body being unable to move properly. My quality

of life is extremely poor and there is very little chance of improvement, more

likely to get much worse.
