We are seeking people with Psoriasis, Psoriatic Arthritis Or

Ankylosing Spondylitis to participate in a telephone interview,

please tell us about your experience receiving care.

The purpose of this study is to better understand your experience

with receiving care for your Psoriasis, Psoriatic Arthritis or

Ankylosing Spondylitis. We are focusing on what went well and what

didn't go well when you received care from healthcare professionals

with respect to testing, diagnosis, referrals, treatment and

management.

If you:

· are Canadian or a permanent resident and 18 years of age or

older

· have been diagnosed with Psoriasis, Psoriatic Arthritis Or

Ankylosing Spondylitis

· have seen a healthcare professional at least once in the past

12 months

· have seen a specialist (Dermatologist or Rheumatologist)

Please note that the number of participants is limited. Interviews

are scheduled according to participants' availability. Once the

interview is complete, participants will receive a financial

compensation of $50.
