Hi Dede, welcome to the group! First place to start is to call Debbie and get

your patient education manual mailed out to you. And to fill out the patient

information form on the web site and get it sent in. Then start on all the

famous page 6 requirements! There are plenty of people on here that are more

than willing to answer any questions you have as you go along in the process.

PLEASE DO NOT CALL Debbie to ask for a manual.

Remember 2 things:
