Guest guest
Hi Dede, welcome to the group! First place to start is to call Debbie and get your patient education manual mailed out to you. And to fill out the patient information form on the web site and get it sent in. Then start on all the famous page 6 requirements! There are plenty of people on here that are more than willing to answer any questions you have as you go along in the process. You are about to start an incredible journey. Welcome and best of luck to you! Bonnie 2-21-
Guest guest
Hi, Re: > Hi Dede, welcome to the group! > First place to start is to call Debbie and get > your patient education manual mailed out to you. PLEASE DO NOT CALL Debbie to ask for a manual. I understand that you would like a copy of our Patient Education Manual. To request a copy of the Manual go to http://www.clos.net/forms/request_manual_form.htm We will try to send you a copy as soon as possible but at the moment we are being overwhelmed with new requests. We spent $3,000.00 on Manuals in the last month!!! The best thing that you can do is download a full copy of the Manual from our web site at http://clos.net/get_patient_manual.htm Remember 2 things: 1. You need a copy of the free " Abode Acrobat Reader " to read and print the manual. The Adobe Acrobat Reader can be downloaded from http://www.adobe.com/products/acrobat/readstep.html and 2. The Manual is Big and will take a while to download to you computer. Please be patient. Also the Manual is only a small subset of everything that is on the Web. There is nothing in the Manual that isn't on the Web. I recommend that you spend your time looking at our web site at http://clos.net Everything that is in our Manual and More! is on our web site...
