Vaginal Pain Survey

This is an anonymous survey conducted by the Women's Midlife Support Network to gain a better understanding of the impact vaginal pain has on relationships as experienced by women in midlife. There are 30 questions. Please try to answer all questions. The collected information is statistical only and no identifying information will be required.This data will be incorporated into a book on the effect of vaginal pain on relationships.
