need help on a letter for revision

I'm trying to get my ins. to pay for a revision to the by-pass (RNY)

so far they have denied me. i have had an edg done and i have gastric

outlet obstruction. i throw up at least 3-5 times a day. i eat no

solid food. i mostly live off of protein drinks now. please if anyone

can help me with a letter or have one i can go by i would be sooo

thankful.
