I don't know if you've gotten a chance to look at Frye and Dan Rossignol's excellent meta-analysis on melatonin in autism that was recently published, but they were constrained in reviewing what had been studied already and by what the authors knew about melatonin who did those studies. There was a lot these investigators didn't know.

Did you know that melatonin can help with reflux?

It is actually a big regulator and participant in intestinal physiology. Some melatonin is made in the pineal gland, but even more (unbelievably!) is made in the gut.

So, you might take a look at the studies, and see if you'd like to try melatonin, and see if that helps.

If, when you get your OAT, you would like me to do the dilution correction that becomes necessary when the kidneys start secreting creatinine (throwing the usual correction off) let me know. This correction can only be done on the full OAT, not the microbial OAT.

One of the evidences that this secretion of creatinine to urine in the tubule cells might be happening is that serum creatinine will go down at the same time, because creatinine is being removed from blood and added to urine. Years ago, when people made creatinine the standard for calculating dilution, they thought it just went through the filter, but didn't think it was secreted. That ended up incorrect.

Abstract

PURPOSE:

Knowledge of transporters responsible for the renal secretion of creatinine is key to a proper interpretation of serum creatinine and/or creatinine clearance as markers of renal function in cancer patients receiving chemotherapeutic agents.

EXPERIMENTAL DESIGN:

Creatinine transport was studied in transfected HEK293 cells in vitro and in wildtype mice and age-matched organic cation transporter 1 and 2-deficient [Oct1/2(-/-)] mice ex vivo and in vivo. Clinical pharmacogenetic and transport inhibition studies were done in two separate cohorts of cancer patients.

RESULTS:

Compared to wildtype mice, creatinine clearance was significantly impaired in Oct1/2(-/-) mice. Furthermore, creatinine inhibited organic cation transport in freshly-isolated proximal tubules from wild-type mice and humans, but not in those from Oct1/2(-/-) mice. In a genetic-association analysis (n=590), several polymorphisms around the OCT2/SLC22A2 gene locus, including rs2504954 (P=0.000873), were significantly associated with age-adjusted creatinine levels. Furthermore, in cancer patients (n=68), the OCT2 substrate cisplatin caused an acute elevation of serum creatinine (P=0.0083), consistent with inhibition of an elimination pathway.

CONCLUSIONS:

Collectively, this study shows that OCT2 plays a decisive role in the renal secretion of creatinine. This process can be inhibited by OCT2 substrates, which impair the usefulness of creatinine as a marker of renal function.

If you don't mind my asking, what signs of adrenarche are you seeing in him?

adrenarche in him. His initial workup was negative for adrenal hyperplasia but

he has a follow up coming soon.

My son is on 3 mg of melatonin which he takes every night. I looked for the

article by Dan Rossignol but couldn't find it so far but will keep looking. I

did read the one that you sent the link for. Once I have the OATs test result, I

will forward it to you. Thanks

Hi Cathy,

My son started having a lot of body odour this year. Also developed two thick

hair in his armpit. Earlier this year he had a growth spurt and when we did the

blood work, his testosterone was found to be high but his androstanedione(which

indicates congenital adrenal hyperplasia) was normal. He has some bumps on his

nose. His hair growth all over the body has increased. When we found out that

his testosterone is elevated, I did some digging and found that too much zinc

replacement will lead to elevated testosterone. Body builders love to take zinc

for the purpose. My son was on 50 mg then. He was also taking l-carnitine then

which potentiates the action of testosterone. So I stopped zinc and repeated the

testosterone level in a couple weeks and the repeat level was normal. He did not

have any further hair growth but still has body odour. His testes are

prepubertal so it doesn't look like precocious puberty. It could be adrenarche

based on some hair growth, bumps on his nose, and body odour but we will not

know for sure till we get the followup done next month. We will have the

hormonal reevaluation and see what it shows. We have a feeling that my son was

growing very fast last year but now his growth has slowed. His bone maturation

was also ten years when he was not even 8 yet. If he does have mild congenital

hyperplasia, that would mean that in times of stress, he doesn't make enough

cortisol and makes other hormones on the pathway instead because of deficiency

of one of the enzymes. Let me know if you have any other questions as I can

provide more detail on adrenal hyperplasia.

Hi ,I have been reading about holy basil and really want to try it. But I personally have an enzyme deficiency which actually leads to low cortisol in stressful situations. Holy basil may actually help but I am not sure of my current hormonal status. I had the workup done several years ago and never did the follow up. You have motivated me to get myself checked out and once I figure out what my hormonal status is, I would love to try holy basil. I think my son is going through the same hormonal imbalance as we see signs of adrenarche in him. His initial workup was negative for adrenal hyperplasia but he has a follow up coming soon. I have heard that the kindest most knowledgable gastro is Dr Buie at Mass

General Hospital. There is probably a massive waiting list but worth an inquiry.
