The following Policy was addresed by Mrs Perkins in 2000. However, during an inspection the individual/provider should be able to produce the card or a copy of it for verification . Maxie Bishop, RN, LP, State EMS Director EMS and Trauma Services maxie.bishop@... Policy: A licensed EMS provider and registered FRO shall assure identification is worn by all of its employees, while providing patient care, and that identification shall include, but is not limited to, the following items: 1. Personnel's initial of the first name 2. Personnel's last name 3. Certification/License level 4. Name of service TDH is not recommending a specific type of identification badge, but the identification shall be prominently displayed at all EMS scenes. _________________________________ C. You have to have a company id and your DL so they can put a face to the name. Re: Cert Cards I, for one, appreciate the clarification.
