Guest guest Posted January 31, 2008 Report Share Posted January 31, 2008 Hello, For anyone on the lists dealing with the billing and new medicare signature requirements, what are you doing when you cannot get a signature...i.e..unconscious patient with no family present, nursing home patient with Alzheimers being transported to the hospital for dehydration,..you get the picture. My billing guys says " YOU WILL get a signature for transport for all medicare patients, no exceptions " ..and doesn't consider unconscious or altered a reason for no signature. Along with a witness signature. Guest guest Posted January 31, 2008 Report Share Posted January 31, 2008 The rule reads that a signature is required except in two cases: One the patient is mentally incapable or Two the patient is physically incapable. In those two cases you must get a provider of direct care to sign, and state the reason that the patient cannot sign. This person is not you or your partner any longer, it is is the nurse in charge of the patients care, or the doctor in front of you. AND your billing person is correct you must get somebody's signature, no exceptions. This meets the requirements. It has given me great resources to enter into further negotiations with him..<G> This is a tiny, rural, county based service with hand written PCRs..they have always required the receiving RN to sign for the patient and we use to be able to write " signature unobtainable " or 'Patient unable to sign " and document in the PCR why. The billing guy felt this was no longer adequate. However, since I now get paid to work here (this has been a part of my volunteer service since I started this back in the days of...well you know), we are moving to ePCRs within the next 3 weeks (change is HARD, let me tell you..<G>) The forms and documents so many of you have shared will go a LONG way in helping take the stress off the crews and something solid to show the billing people!!
