Emergency Medical Services for Children (EMSC) Survey

Dear EMS Agencies and Hospitals:

The EMSC survey discussed at the November 2007 GETAC meeting and EMS

Conference is now up and ready for EMS Agencies and Hospitals to

complete.

DSHS has a grant from the federal Emergency Medical Services for

Children program that requires a survey of all entities in Texas

providing emergency medical services to children. Your answers may help

to identify areas for improving emergency services for children in our

state. In addition, your response allows Texas to remain eligible for

future funding. Please be assured that the answers you provide in this

survey will be confidential and reported only in aggregate form. The

deadline to complete the survey is March 15, 2008.

URL for SURVEY: http://www.nedarc.org/surveys/TX/

The link provided in your entity's emailed invitation or the above URL

can be used to complete the survey. You will also need your DSHS license

number or EMS/Trauma Registry entity number. We need all Hospitals and

EMS Agencies to complete this survey! Reminders will appear in the Texas

EMS magazine.

Contact Beverly Willis if you have questions or to complete the survey

by paper. x2173 or beverly.willis@...

Thank you in advance for taking time to complete this survey.
