One is the letter assigning your EIN and the other is a notice letter sent after the second notice from IRS on backup withholding. Both those come to you from the IRS. You should be talking to the IRS about this. It helps if you have a CPA involved who does this sort of work. But you should be able to resolve it by talking directly to the IRS. Have any of you received a leter from FOX Systems, Inc, the CMS Enumerator, that states a Business Name/EIN mismatch? We are a City Department, all licenses and registrations are as " Seminole EMS " and has been so since before I came to the system. The EIN is for " City of Seminole " , which we are a Department. FOX Systems has stated we must provide IRS CP-575 or 147C in order to match the service name and EIN. Has anyone else received this letter? If so, are you having difficulty obtaining the documentation? Any suggestions? I am working with City staff on this as well as the City Audit firm, but so far the CP-575 can't be located. Anyone know the process for 147C. There is a 15 day time limit before automatic deactivation of NPI occurs with possibility of penelty. They must be getting many calls in regard to this because the IRS contact knew what the situation was when told the call was in reference to a letter from CMS and FOX Systems. Guest guest Posted June 17, 2008 Report Share Posted June 17, 2008 , One of the issues we had when the NPI went official and we got a ton of denials was that our EIN had the dash in it after the first two numbers....we got that removed, turning it into a single 9-digit number and it fixed our issue.?
