Speaking hypothetically, it would seem that the next alternative to Narcan in some cases would be intubation. In my opinion, the problem is not Narcan.? The problem is in paramedics who give Narcan without understanding its indications (IE, the coma cocktail is as dated as bell bottom jeans) and who don't titrate Narcan to maintain respiratory status.? We shouldn't be using Narcan to take away a patient's high or as a diagnostic tool.? Narcan should be used only to reverse respiratory depression secondary to a suspected opiate overdose. 0.4 mg of Narcan may be too much in some cases, but in other cases, it may take 2.0 mg of Narcan to do the job.?? Until we get away from cookbook medicine, we're going to continually face gaining and losing medicines in our formulary on a semi-regular basis. Going with a generic dose for everyone(as has been done in the past) makes about as much since as continuing the idea of giving an " Amp " of any other drug. These are old school mentalities and while yes I was trained in the old school way all it tkes is keeping up with changes. People wonder why as a proffession we cannot advance and dont get the respect we feel we deserve. Not being willing to grow and move forword is the reason. EMS and Fire seem to be the biggest culpret of saying " well back in the old days we did...
