RE: Reporting employment to DSHS?

I know that EMS services are required to report a complete personnel list during

each relicensure process. I thought that was where the state got their info as

to where you might be employed or volunteering. However, that list is only

reported at 2 year intervals. I know of no one who reports where they are

working to DSHS individually.

I happened to notice that the back of my DSHS license gave an address to report

employment to.? I was unaware of any requirement to report employment to DSHS.?

I've checked the Texas Administrative Code and seemed to strike a dry hole.Does
