If you or anyone you know needs to fill out and sign an ambulance MOA or

a personnel MOA with DSHS for possible deployment over the next few

days, here are both documents.

The ambulance MOA has two options. The first option requires you to go

to all requests for State Facility evacuations. If you sign this, you

are saying you will go evacuate a facility no matter what if they call.

The second option is one that says if you are called to go on a State

Mission, you MAY go if you can do it and not harm your home

jurisdiction.

The second MOA, Personnel, provides a mechanism for your personnel and

other vehicles (Strike Team Leaders, etc) reimbursed if you send them.

This MOA is crucial for Strike Team Leaders, EMS Commanders and ALS Bus

Medics...this even applies to VOLUNTEER agencies who may not be able to

send ambulances but has personnel that would like to participate.

Please consider signing one or both of these and fax them to the numbers

below. This will allow you to get deployed and have guaranteed

reimbursement once you get back home. We are potentially in need of a

large number of ambulances and EMS Personnel for evacuation and

post-storm response. Anything you can do to help will be beneficial.
