Thats great nne he sounds like a great doctor. I hope Dr Goldstein (who is closer to me) does this type of exam as well. It kills me, it seems like none of the gynes take steps to rule out things like tumors, etc. I remember my gyne in Pittsburgh wouldn't give me an MRI I had to ask my PCP whats up with that?

grrrrr!

Carolyn – that is a great article. And the exam is exactly

what Dr. Conway does. Remember I told you his exam was nothing like I had ever

had. That is one of the ways he can tell if it is PN. Especially the part about

the “anal sweeping”. Rectal and rectovaginal exam are

performed last. With rectal examination as one starts in the posterior

midline and sweeps laterally and anteriorly, the rectal finger passes over the

piriformis, the coccygeus, and then the levator ani muscles. Rectal examination

should also include evaluation for rectal masses.

You wanted to know what Dr. does in his exam and this article is

a perfect example

Dr Weiss did too.

I'm sure he was evaluating me during our interview in his office and then I remember that he had me walk first to the exam room without telling me why. But of course now I know he was evaluating my walk. He also performed a gyn exam like none I had ever experienced. No one had ever bothered to check my muscles and nerves. It was so wonderful to have a doctor who GOT it. I remember saying to him "so you don't think I'm crazy?" And he said, "of course you're crazy, no one can be in this amount of pain and not be a little crazy." He BELIEVED me.

I remember watching Dr. Conway's powerpoint presentation and he was saying that he performed a routine pelvic surgery on a girl and her pain got worse instead of better. And what did he do? He tried to help her. He did research and got educated and is now an expert on pelvic pain. A damn site different than my jerk surgeons who each took a shoulder and escorted me out of their office - yes, through a crowded waiting room. And they told me to go straight to a psychiatrist and to NEVER call them again because their surgery was a success and that's all there was to it. Of course I must be crazy.

So there is a standard for which to strive.

I don't know if you know, but I was seriously messed up with pelvic pain and interstitial cystitis. I was in horrific 24/7 pain and suicidal. I am now well thanks to PT and several nerve blocks (perhaps unnecessary, I will never know, but I'm not complaining).

If you all like that article you will also like Dr. Weiss and Prendergast article Screening for Neuromuscular Aspects of Pelvic Pain. They are actually strikingly similar.

It is in the files section of happypelvis, perhaps chelle could down load them here or I can.
