This was the questionnaire that was sent to join the class action. Also, I don't have the number on me, but if you do a google search for "disability rights florida", you should come across their website with contact info. I would recommend calling them before you fill this out. When I spoke to them a few months ago, she did warn me though that this could take years to resolve.
