Guest guest Posted September 2, 2008 Report Share Posted September 2, 2008 I'm posting this at more than one site, simply because I have found help from many of you and want to pass it on. Tonight I went to the movies in capri pants. Not even real baggy capri pants. And hey, I was fine! I still have some pelvic floor issues, but next to zero vulvar burning. First, let me say that the remedy I'm suggesting might not work for everyone, and second, for all I know it would not work for me if I had not done a lot of other things over the last three years that got me to this point. I won't go into all of those things, just summarized by saying the spot of dermatitis is long since healed and the spot where they did a biopsy also, meaning my vulvar area tissue is in good shape. I have been on Lyrica (1 year) and now on Amitriptyline for a year, use a " butt pan " to rinse, but with much less frequency now, and have used a topical Amitriptyline/Baclofin which is soothing. I have had a ton of PT (by an experienced PT and once with Rhonda K) and probably need more, for the deep butt muscle issues. I have had my share of UTI/yeast/acidophilus/yadda yadda. Anyway, I had read about Capsascin for a while and my specialist at the U said it sounded harsh, so I didn't pursue it. Since I have left the specialist for my GP (well, not in the Biblical sense, but I dumped the specialist) I talked with him and his medical student about Capsascin recently. The med student was really curious about the symptoms I have and interested in my fat and bizarre history file, bless his heart. When I brought up Capscasin he brought up the neurotransmitter Substance P and we both lit up like light bulbs. I know excess substance P is one thing they have found runs in families when women who are related have vulvar pain. He knew that Capsascin is used to reduce Substance P. So, as nuts as it sounds (putting hot pepper extract on the part of your body that already burns) we decided to try it. The first " prescription " was filled by the pharmacist with OTC Capscascin product. I turned it down and got the GP to prescribe Capscasin in an acid mantle base, .025 percent, per the instructions of Hollis and other women on these message boards. I tried a teensy bit on a q-tip, just on the little place the biopsy was taken, because that always seems like the main source of burn.. I hardly felt it. The next day I used about half of a pea size amount. WOOOO. It burned like crazy, which kind of scared me, but I waited 20 minutes and then rinsed it off and VOILA. I had a darned good day. My process now is to put the stuff on in the morning, hang around standing up for about 20 minutes, use the plastic butt pan and some stuff called " Sweet Spot " unscented liquid soap (PH balanced, which is important to me because of UTI/yeast issues) to wash, pat dry with a clean dry wash cloth (I have a colorful stack of terry cloth washcloths in a basket on the back of the toilet and a basket to throw them in as i use them) and then with a tissue, put on a little of the left over Amitriptyline/Baclofin plus a little 5 percent Lidocaine (the 2 percent is totally useless, I don't really get much out of the 5 percent, but I might as well use it up) and I'm good for the day. Still some butt aches, but not that burning feeling when you sit at a computer or wear pants. Of course, I still don't push it. if I can wear a skirt, I do. And I don't sit for long periods even if I feel okay. I think I still need some PT, body work, or maybe evaluation for pinched nerve, but I am trying a few things on my own in terms of self massage and excercise before I spend more time and money. And the urgency is not high, considering how minor the pain is. It isn't even really pain.
