:I wanted to ask you did they say atypical or atypia? The reason I ask is that many times when I had my ERCP the dr said that is was clear, and when I saw the report it said atypia. My understanding is that atypia is the most severe form of inflmation. I believe that every person with PSC has atypical findings in their ducts as inflamation should not be there. Unless I missed something in your email. Was the Dr that did the ERCP concerned?

Atypia is a clinical term for abnormality in a cell. The term is medical jargon for an atypical cell. It may or may not be a precancerous indication associated with later malignancy, but the level of appropriate concern is highly dependent on the context with which it is diagnosed.

An " Atypia " ERCP means it's not typical (with most PSCers, bile duct abnormalities). Atypical cells means just what it says. The (currently inactive) colitis portion of my colon has exhibits atypica due to the diffuse look of the blood vessels ( " blushing " rather than clearly discernible vessels); the polyps they may remove are atypical.

Hi Arne!Thank you very much!When I had asked my dr about the atypia. He explained to me that the inflamatory process has damaged the structure of the cell, hence rendering a diagnosis of atypia.My semi educated guess is that dysplasia is precancerous, and differs from atypia. Albeit that many forms of dysplasia were at some point atypia.I have seen one instance where the local pathologist diagnosed the specimen as 'low grade dysplasia' and recommended a colectomy. The patient went for a consult to a second Gastroenterologist who sent the specimen to the AFIP (supposed to be an elite lab) discounted the first diagnosis and redisgnosed it as atypia and recommended leaving the colon alone.Just my two cents:-)ShaulUC PSC
