Open Position - Care Coordinator

A local South Florida provider is seeking to fill a Care Coordinator position within our Developmental Disabilities and Autism Services division. The position is responsible for coordinating all patient encounters and providing support service to ensure the best care possible. 

Duties may include: 

managing patient database

providing clinical service education

scheduling appointments with field clinicians

collecting or updating financial information

investigating pre-authorizations / approval for insurances

establishing systems to improve the patient experience  

maintaining confidentiality / HIPAA

Coordinators will be responsible for regular contact with patients to assess and facilitate ongoing care needs.  This is a full time, salary based position located in Pompano Beach, Florida.  

Position Prerequisites:

Bachelor’s degree, preferably in a health related field

Excellent verbal communication skills

Professional level of experience in a education or health care field (e.g. teacher, registered nurse, therapist, etc)

Knowledge of complex conditions such as Autism, Alzheimer's, etc.

Proven track record of health, education advocacy experience

Strong technical proficiency, ability to work in fast paced environment
