Hi everyone,

has anyone experinced low Albumin level? what have you done. Also what would cause Alkaline Phosphate to just double? My lab work showed that it doubled -1200 range. My AST and ALT went up also a little 136 adn 173. My albumin came down half... from 4.8 to 2.4 So what would cause my AK to double and AST and ALT to go up.

Low Albumin can be realted to portal hypertensiona,cirrhosis and

ascites in people with liver disease.

ALP can increase greatly increase in both extrahepatic and

intrahepatic obstructive biliary disease and cirrhosis or also a

liver tumor. Some drugs can elevate ALP too, allopurinol,

antibiotics, verapamil and more. Also a healing bone fracture can

increase it. this is paraphased from Mosby's diagnostic and

Laboratory test reference book.

Have you had a recent MRI or does your doctor have other test ordered

for you?

Hi Arman;

I am sorry to say that the decline in your serum albumin is a bit

troublesome. Albumin is made by the liver, and so serum albumin tends

to fall as the liver fails. Serum albumin, age, bilirubin, variceal

bleeding, and aspartate aminotransferase are the main factors that

the Mayo group has been using to assess prognosis in patients with

PSC:

If you do not have elevated bilirubin and have had no history of

variceal bleeding, and have a normal international normalized ratio,

or prothrombin time (a measure of blood clotting ability), then it is

possible that the low serum albumin could be an artifact (in which

case you could request a repeat measurement to confirm it?) or, it

could be due to other factors. For example:

" In general, albumin is a good marker of severity of chronic liver

disease, but levels may be affected by chronic renal insufficiency,

urinary protein losses, or gastrointestinal losses. "

Your elevated alkaline phosphatase and elevated AST and ALT are

probably the result of bile-duct blockage(s)/cholestasis:

" Markedly elevated levels of ALP suggest the possibility of such

disorders as extrahepatic biliary obstruction, primary biliary

cirrhosis, drug-induced cholestasis, primary sclerosing cholangitis,

and infiltrative processes (eg, amyloid, granulomatous hepatitis,

neoplasm). "

So, it might be good to have an MRCP or ERCP to assess whether or not

such blockages could potentially be alleviated by endoscopy?
