Vulvodynia is increasingly recognized as a cause of sexual pain. Aim. The goal of this Continuing Medical Education article was to provide a comprehensive review of vulvodynia including terminology, possible etiologies, and offer treatment options. Methods. A Medline search was conducted using several terms related to and including the terms vulvodynia, vulvar vestibulitis, vestibulodynia, and pudendal neuralgia. Results. A thorough review of vulvodynia. Conclusion. Vulvodynia most likely represents several disorders without an identifiable cause in many cases. The management of these patients requires a sensitive provider who can coordinate a multidisciplinary approach to their care. Despite the lack of large-scale, placebo-controlled trials, several new treatment options exist. Everytime I make comments about a study, etc., people choose to determine that I am making war on the person who sent it. I never do that. I do however think we should be critical of this stuff and not just swallow everything they print. Send us more. It is great fun. Personally I would make a point to stay away from that particular health center. Background: The objective of this study was to describe the nature and number of pain diagnoses in a chronic pelvic pain population, and to determine the relationship between these and a patient's history of abuse, drug-seeking behavior, or initial symptom scores. (My Opinion)(Snark) This will really produce all kinds of good results which will no doubt lead to a fast cure for all the chronic pelvic pain population. The diagnosis of drug seeking was based upon a patient's refusal of non-narcotic management. (My opinion) Apparently girls, if you refuse to take an aspirin because you know by experience (or by the intensity of your pain) that it won't help, you are considered a drug abusing, drug seeker. Kewl! They used to be polite enough to just call you crazy. Now you are a drug addict apparently. (No insult to drug addicts by the way.) Some of the nicest people I have known have used a lot of pain relieving drugs and more power to them). Conclusion: Patients with chronic pelvic pain frequently have multiple pain diagnoses, and non-gynecologic pain in particular is common. Multiple pain diagnoses and a history of abuse have similar symptom patterns, suggesting a relationship which can be used to distinguish these patients from those with drug-seeking behavior. (My Opinion) If I am correct in my interpretation of this wondrous piece of research they are saying if you have more than one pain and it isn't centered in specifically the anatomical region of the gyn's specialty you are under suspicion of being a drug seeker. (Of course we are drug seekers if we are in pain just as this messed up system has taught us to be because they never look for causes but only for the next pill or ointment.) Of course I could be wrong? Beam me up, y.
