Please go see your specialist. I too have severe clitoral pain and it

is due to lichen planus of the vulvar. The white patch may be lichen

scleroisis or just what they call smagma, something like that. It

should come off if that is what it is. If it is ls, ;you will need to

seek treatment. Is that the only area that has the white patch?

Good luck,

Thank you for your response. Yes, that's the only spot, and it won't

come off. I did read about smegma, but it seems too adherent to be

that. I made an appointment at a new gynecologist (does anyone know

Dr. Eschenbach in Seattle?), but I have to wait 3 weeks. Ugh.

Though my other VV pains are definitely nerve-related, I'm beginning

to think that the clitoral pain might not be. My pain is at the very

tip of the clitoris, and always has been. It always feels like there

is a thorn in it.

I have white discharge that comes out of my clitoris. This is a

chronic problem for me, the entire vulva becomes covered with this

discharge and trying to wipe it off causes further irritation. I

haven't been diagnosed with vulvodynia yet, I have chronic

vulvovaginitis of unknown cause. In my case the white discharge is

actually white blood cells coming out.

I have clitoral pain as well and never thought it was

due to lichen planus or sclerosis, but am now

beginning to consider it. I always thought it was

nerve related, but I know from other people with

clitoral pain that pain that feels like shocks, sharp

pain, burning, etc can also come from lichen. I'm not

saying that that's what yours is, but it's definitely

worth having a specialist take a look at it and at

least if it is you can start treatment, and if not,

you can rule it out. I really thought mine was from

pudendal neuralgia, but i'm just not too sure anymore

for several reasons including: i only have clitoral

pain, it only hurts with direct pressure to the

clitoris or prolonged sitting, pudendal nerve blocks

have not helped, etc. I'm trying to get an appointment

with dr. elizabeth stewart in boston, so hopefully she

can tell me if it is lichen or not. definitely have a

specialist take a look at it though. please let us

know what happens and know that we are here for you!

Yes, I had that once or twice. It hurt and freaked me out, but ended up being no big deal. It's kind of gross, but for me it turned out to be kind of like a build-up underneath the hood, almost sort of a pimple or something. If you go to your doc, she should be able to clean it out with a q-tip or something. To: VulvarDisorders From: natmsels-forums@...Date: Wed, 30 Jan 2008 20:59:07 +0000Subject: Clitoral pain

Hello, ladies. I've had multiple types of vulvodynia for over 20

years, which is somewhat controlled with antidepressants. One of my

pain types is clitoral pain, which nothing seems to help. Lately the

pain has increased, and today I noticed that there is a small white

mark (plaque?) under my clitoral hood, adjacent to the clitoris. It

won't come off. The pain seems to be the most intense there. Has

anyone else experienced this? Perhaps it is a form of lichen?

I have not been to a vulvodynia specialist for many, many years and

have recently moved to the Seattle area from Philly. I think it is

time to find a new physician and get re-evaluated. I'd also like to

get off of antidepressants if I can, though I think it is my only hope

for the generalized vulvodynia.

Thank you all for sharing your stories. I read this forum all of the time.

So you had the privilege of undergoing nerve blocks, too, huh? I had

pudendal nerve blocks done many years ago, plus an epidural, and the

clitoral pain wasn't affected at all. It was very upsetting to get no

relief and traumatic to go through those horrible procedures.

Maybe now that many years have gone by since I went through a raft of

treatments (nerve blocks, biofeedback, releasing the clitoral hood,

painful rolling physical therapy [internal and external], drugs, etc)

the physicians will have better knowledge about my problems. I hope.

Hi - reposting something Dee had sent... this helped me A LOT. I can clean my clitoral hood with no pain now and at first it really hurt.-----HI all....

Seeing someone mention clitoral pain this might be a long shot but maybe not. This is something (below) that I've sent before on the Clitoris and keeping it clean and it's something that generally we don't think about 'unless' we've had problems, so for what it's worth.

Dee~ ;)

=============================================

HI all, Thought I'd add this about how important it is to keep inside that clitoral hood area clean just as it is for men esp. those who aren't circumcised.

Here's some information about smegma, (this is probably more than anyone wanted to know), but there are glands on the inner hood of the clitoris that secrete an oily substance called ''sebum'' that lubricates the clitoris naturally (so it can slide).

And by the way those ARE the same glands on the inner surface of the males foreskin so that it slides too. In fact it's what makes the head of the penis shiny (same as our clitoris itself is shiny) and when that oily substance (sebum) accumulates it's called 'smegma' and it can have that cheesy odor or stinky feet, *chuckle* but that is a totally distinct odor and usually not an infection.

Not only the smegma can get built up under there but if that area is not cleansed well, think about these things!

The hood or shaft may trap sweat, or vag. secretions, medications, semen, toilet paper fibers, pubic hair, menstrual blood, lubrications, urine, and possibly feces if we wipe backwards. All of that if not kept clean can lead to bacterial infections. (not that it will but may)

So it's easy to understand why if there were adhesions inside that hood area where it can't be slid back & forth, it's 'not' a good thing to have and if one does have pain in that area it may well be a cause of it, as even the smegma itself can harden into tiny hard pieces that would irritate if trapped in there. (sort of like hard ear wax)

I never gave all those things a thought, 'before' I learned all of this years ago... but I make sure I keep that area clean now. One thing I do (now & then) and use my meds there is what I've named the Q-tip 'twirl' many years ago *grin* and you may see some members refer to that.

I've been doing this for a long time, and that's to put a tiny dab of the meds I use (the E & T) on a Q-tip and twirl it inside the clitoral hood or shaft. (even just vaseline or the Emu oil would work too if meds aren't needed there).

When it's loose and fine, the head of the Q tip can go in most of the way 1/3rd inch? maybe? and no it can't get lost. *smile* I did have someone ask me if it could go into the body, nope it's a dead end, a cul de sac. *chuckle* .. ;)

But do make sure it's well lubed or wetted with the vaseline, emu oil or the meds, so it doesn't cause friction and just twirl that in one side and over the clitoris itself (inside of the hood or shaft), sort of like rolling over a speed bump. It keeps the hood loose from the actual clitoris so that it can be tugged back just like a penis. (of sorts *chuckle*)

Keep in mind that some women are fused or have adhesions there and it may be the barest of dimples... and no way would a Q tip go.. but ''gently'' working on that can release it and I've had more than a few tell of the amazing release that was just a dimple at one time as they had no idea that shaft could or should be pulled back to keep it clean inside.

'I' think it's probably easier to clean that clitoral area with a bath and a soft washcloth or really just our hand, and even by soaking it can help keep that area clean. It's probably easier in the tub sitting there & gently pulling the hood back than to do it in a shower. Luckily I'm one of those who likes a bath and I know, I know, (grin) many prefer the showers but not me. It's my only time to relax, LOL

Plus 'I' take my baths at night before bed, not in the morning. I like to be clean when I 'go' to bed rather than the dirt of the day being there and getting one in the morning, (but that's me) and esp. ready for love making plus it keeps the sheets clean, LOL A shower is just too fast to really warm up the skin for any length of time (I think) and my doc thinks baths are ok (good thing, I'd do it anyway) *grin*

Like I said probably more than anyone wanted to know, so for what it's worth. :)

I have lichen planus that affects the clitoral area, and there is just

a different look between smegma and the white plaques of lichen.

Smegma, although often very difficult to get at, should at least be

kind of moveable.
