The studies on urso are conflicting as to benefit, with the exception that it appears to provide some colon cancer protection (for which PSCers are at increase risk). It may delay the time to transplantation, as it clearly does improve liver chemistry. It IS a benefit to PBC patients. The rest of the regimen is consistent with most hepatologist's recommendations. There are studies that show benefit from milk thistle (see http://www.psc-literature.org/scilit.htm under Milk Thistle), and some that don't. I take milk thistle and SAMe (see above source, under SAMe) - both AFTER my hep OK'd it. Most people with PSC do better with low fat, low (animal) protein diets. Minimal or no alcohol is also usually recommended. If you have UC or Crohn's, it's pretty important to pay attention to diet, as you may not absorb sufficient nutrients otherwise. I consider it more an additive (imitation Bear bile) and for me I have no side effects. For the first tens years of my disease I wasn't on anything but my liver enzymes slowly rose, but for the last eight years I have been on Ursodiol and have better Liver enzyme readings than in the first ten years.
