OT. What do all the numbers and letters mean

I'm fairly new to the support group and wondering what do all the numbers and letters at the end of e-mails mean

ie.

Texas - Together in the Fight, Whatever it Takes!

Son

Ken (33) UC 91 - PSC 99 Listed 7/21 @ Baylor Dallas "

I understand some of it:

- Son Ken

(33) - her Son Ken is 33 years old

- UC 91 - he was diagnosed with Ulcerative Colitis in 1991- PSC 99 - he was diagnosed with Primary Sclerosing Cholangitis in 1999

but what does " Listed 7/21 @ Baylor Dallas " mean??
