If they won't do the CAT with the Val Salva, at least have them do it with inspiration and expiration. Just about every place can do that even if they're puzzled by the Val SAlva ( actually a Val Salva is just to take a deep breath in and hold it and bear down as if you're going to have a bowel movement -- of course you don't move but it increases the pressure inthe chest). So it shouldn't be aproblem for them to do all of those really. It's just like you say, for them to listen and take the time to understand. Good luck! Mythoughts are with you (and the force too!)
