The Alchohol certainly helps to keep down the stink, but it helps a lot too to make sure that you use a really strong smelling shampoo on his hair. L'Oreal Kids shampoos or Suave kids have some really nice scents that help the baby's head to smell better and also the smell will often somewhat transfer to the foam in the band too. I'm not 100% sure of what other orthos recommend, but CT said to use alcohol and an old washcloth (for the rough texture) or soft toothbrush. Also for stains (cause it can get pretty dirty), I was told to scrub it with a dry washcloth first, to get that texture in there to scrub at the layer of gunk. I usually use a toothbrush and scrub three times each night (all at once). Also, once you start getting some adjustments, about half of the stinky parts get scraped out and it isn't as bad. Another tip they suggest is placing it in the sun to dry. Our ortho told us not to scrub the dark spots too well as seeing the spots showed her where the baby's head was making contact with the band and therefore where it needed to be shaved down during an adjustment (get the band clean but not so clean that you can't see the spots).
