Actually, this *is* what can cause spotting! My GYN told me that spotting and even heavy break-through bleeding are quite common in situations like this, because your body's natural cycle is (roughly) 28 days and your body can take a while to adjust to being regulated otherwise. When I started taking Seasonale (a 3 month birth cycle) I had heavy bleeding for a little bit over a month. The second cycle I had it for a few weeks. As my body adjusted it got better. Now I've been on it for a few years and most cycles I'm fine, but sometimes I still experience a "period" that is a few weeks long. I'm not sure how long you've skipped the placebo for, so your body may just need time to adjust? Otherwise, if it's been a long time, it may be time to speak to your doctor about coming off, having a period, and then going back on again. I had a pelvic ultrasound about 6 months ago, and they didn't find anything. My other symptoms include severe breast pain, which started before I started the birth control. Vulvodynia.
