I had some success with Botox and PT. After the injection, I stepped up my exercises and self-massage, doing it for about 3 hours each day. A month later, I do feel a bit better. It wasn't a night and day difference, but after many treatments completely failing, I am happy to find something that results in some improvement.

I want to emphasize that so far it has been a slight difference, and it took a couple of weeks for me to feel any difference at all. My doctor said something along the lines of the muscle being fine (floppy was his exact word), but I got the impression there's some secondary action of the botox that takes a bit longer.
