Dear Families,

JARC is committed to developing new, innovative, affordable housing options for adults with disabilities.

We need your and your child's input in determining the needs and challenges that exist in obtaining the least restrictive housing.

Please fill out the attached questionnaire and return it by Monday, January 24th so we can quickly move forward and continue doing things with people with disabilities, not for people with disabilities.

This questionnaire is quite thorough and asks for detailed information so we can best assess community needs. If there is a portion you are uncomfortable filling out-please leave it blank. We will do our best with the information provided.
