Guest guest Posted March 2, 2004 Report Share Posted March 2, 2004 Hi Zoe I live in a town where there were 3 NHS Health Authorities at the time of my Resurfacing i.e it happened to be on the border of 3 authorities - These were South Derbyshire, South Staffordshire and North Staffordshire. I initially saw an OS at a hospital in the South Staffordshire Authority who advised me to have resurfacing. At the time (November 2000) he and no one else at this hospital did resurfacing so he referred me to another hospital in the Birmingham Health Authority. All this was done with no problems whatsoever and all I had to do was turn up for the appointment. I was told by my new OS (Ronan Treacy) that they would get me in on the urgent list as I was bone on bone and in a wheelchair. I stated that I could turn up at 24 hours notice and he got me in under 4 months. Now the patient charter coming in (already part implemented) allows for this under certain circumstances as per a patients right. Implementation dates being arbitrary. I don't believe that I'm lucky - I deep down think sometimes the input by your GP has a lot of sway. Ask for a referral to the Birmingham Royal Orthopaedic, or wherever you choose, in a nice manner explaining your reasons. A number of patients were from other Authorities whilst I was there including a man from East Anglia who had severe arthritic joint problems. He had been operated on all over the country - he said he relied on cancellations in order that he could get the surgery. I can't be certain and think he'd had 20+ operations but I remember him saying that he'd rarely waited more than 3 months. I know I'm repeating myself but speaking to your GP could help. If you e-mail the manufacturers of BHR (mail@..., www.midmedtec.co.uk) they will advise about the most experienced surgeon in the Sheffield area. I've also seen posts on here about surgeons in the north of England, so I'm sure you'd get some advice if you post a request on this board. A good GP should ask you which surgeon you'd like to see (mine did). S/He should then write a letter to the NHS Trust explaining that the complications in your case require you to see a specific surgeon. Beware NHS admin though. My application for out of area funding was lost for 3 months - despite regular calls from my GPs office and me. Of course you may still find that the waiting lists are too long. On a brighter note! When you move you'll get the option to stay with the same surgeon or move to a more convenient hospital. My sister still has regular checkups with her (NHS) surgeon in Hertfordshire even though she now lives in Dorset. Find a name of an OS with a good reputation/record of resurfacing who does NHS work 2. Get your GP to write to that OS 3. Wait for a reply You will most likley get a letter from the OS's secretary suggesting an appointment to establish if you're a suitable candidate for resurfacing. At the examination (most likley new X-rays will be taken unless you bring very recent ones with you), the OS will tell give you her/his opinion, and if positive (i.e. you are a suitable candidate and s/he is prepared to resurface your hip/s) you'll be given the approximate waiting time (as far as I know all Health Trust now undertake to process hip patient within 12 months of placing them on the waiting list latest; many even offer shorter waiting time of 6 months only; by the way, according to recent developments you can insist on being processed within a certain time-frame and if your Health Trust can't meet the Government standards it has to pay for your op privately, including sending you abroad - this may be the best option for you as there appear to be no OSs in your area that perform resurfacing and you could apply to the Health Trust to send you to Belgium where this intervention is even cheaper than in the UK, I understand). Lastly, you can mitigate the waiting time by agreeing to accept sudden cancellations (I benefited from a cancellation and had my resurfacing done 3 months after seeing the OS). Hope this helps. I booked a private consultaion with him at the Thornbury Hospital, Broomhill, Sheffield. He took one look at my range of movement and said " we will have to do something about that " and put me on his NHS waiting list no questions asked. I came under the Wakefield NHS Trust and there was no issue at all as to whether my NHS Trust could offer the operation or not. What I liked about him was that he did not put any pressure on me to have the operation done privately and was more concerned to reassure me that I would still be a candidate to have the operation in a years time. I chose Ian on the recommendation of my cousin who is a consultant (heart transplant team though not orthapedics) at the same hospital, he had no second thoughts about recommending Ian. A year ago Ian was the closest surgeon listed on the BHR device website followed by a surgeon in Manchester. Because I saw him privately I had to wait 17 weeks to go on the waiting list, I then received a letter (September) guaranteeing surgery within one year. I understand that his current waiting time for a consultaion is less than 17 weeks so it is more beneficial to have a NHS referral. If anyone else has had BHR with Ian I would be grateful to hear from them. Guest guest Posted March 5, 2004 Report Share Posted March 5, 2004 Dear all, Today I went to see My GP to ask if she would refer me to see MR.Treacy in Birmingham. At first she said that our local health authority had such a funding crisis at the moment that they had cancelled all elective surgery for the rest of this year(I'm assuming she ment financial year April 5th) she also said that the referrals she had done for patients to Leeds had been 'turned back' by Leeds as their waiting list was so long they were only doing operations in their Local area. I explained how important it was for me to stay active and not have to take things easy for the rest of my life. She didn't agree with this and said I should remember that I damaged my hip badly in an acccident and to stop pushing pushchairs up hills etc. She also read out what my consultant had written in December when I saw him last. He said that a hip replacement was not indicated as yet. This was why she said she couldn't refer me and my OS whould have referred me to see a BHR specilliat if it was right for my situation but probably wasn't. I explained that THR was not indicated because the consultant wanted to wait until the very last moment due to my age and non-sutability for a THR. At first she said that our local health authority had such a funding crisis at the moment that they had cancelled all elective surgery for the rest of this year(I'm assuming she ment financial year April 5th) she also said that the referrals she had done for patients to Leeds had been 'turned back' by Leeds as their waiting list was so long they were only doing operations in their Local area. I explained how important it was for me to stay active and not have to take things easy for the rest of my life. She didn't agree with this and said I should remember that I damaged my hip badly in an acccident and to stop pushing pushchairs up hills etc. She also read out what my consultant had written in December when I saw him last. He said that a hip replacement was not indicated as yet. This was why she said she couldn't refer me and my OS whould have referred me to see a BHR specilliat if it was right for my situation but probably wasn't. I explained that THR was not indicated because the consultant wanted to wait until the very last moment due to my age and non-sutability for a THR. I suspect you should have outlined to your GP that a referral letter bears no costs to her budget (bar 5 min dictation, plus sundry expenses for an envelope and a stamp). Nor does the examination by Mr , which will be charged to his local NHS Trust. Nor will the surgery, if approved, as again this will be performed outside your NHS Trust. And, as far as I know (, please confirm), there's no cross charging between NHS Trusts. If I'm right, your GP's stance was at best ignorant and at worst unreasonable. But, it seems, you may have to fight bureaucracy, at least in your area. Luckily, I was spared all that by a combination of an undeterred GP, a resolute Orthopaedic Surgeon (who warned me at the start that we may hit some funding obstacles and was assured by me that I relished the prospect of facing such a challenge – however, these never materialised) and my willingness to take on the system should this prove necessary. I think that you are in a strong position as the present Government are still trying to convince Joe Public that their investment in public services are yielding benefits (don't forget CAB, your MP, local media etc. which can all be at your disposal). It isn't less prevalent in any country and it's just as resistant to antibiotics in the UK as it is in Belgium or the USA or anywhere. The people that carry it can do so without knowing most of the time. Hospitals in the UK as elsewhere are stringent on testing and isolation - I should know as I was put in isolation at Stafford Hospital with visitors made to wear gowns and masks - the reason: I had been an in patient of another hospital in the previous 6 weeks - I was subsequently cleared. My mother had the same treatment as she had been into 2 hospitals within a short period. If you are found to be a carrier your records are marked and you will be thoroughly checked and isolated should you ever be an inpatient again. The average patient doesn't see this. All hospital workers are regularly checked as are patients on the pre med. The risk is the same across the world. Every country has good hospitals and some not so good. The press over here tend to make matters worse. Guest guest Posted March 8, 2004 Report Share Posted March 8, 2004 Thank-you for your email, I agree Sue the NHS is certainly in a dire situation and I've seen some shocking things done to others and myself due to the lack of quality staff or other resorses. Around xmas time my partner was staying for a short time in the local psychiatric hospital, when his injured arm started to give him such severe pain he requested to see a doctor re this 3 times a day, for over 10 days. When he left the ward he went to casualty, where he was turned away and thrown out by security when he demanded to be treated for this pain in his arm, they hurt it futher in the process. It wasn't till a few days after this that he saw an OS who diagnosed the septic arthritis that needed to be operated on imediatly, then again two day later, both times under a general. He is recovering nicely now and although quite shacken still, getting back to normal now. I have lost faith in our health system and would love to go to Belgium but being both on benifits with kids I dought this is realistic. I'm very amazed and proud that we have got through this crisis, which has been going on for over a year, since this initial accident. I did eventually find a website but your GP always has to refer you and he/she will have to apply to the local NHS Trust for permission to send you out of your area and this will be totally resisted if they think you can be 'fixed' in your own area, even though it means a long wait with your condition is likely to be deteriorating daily. Most of the OS's who are experienced in hip re-surfacing and have a good 'rep' have waiting lists of 2-4 months for you to be seen and then 2-4 months to have surgery privately. He is recovering nicely now and although quite shacken still, getting back to normal now. " Dear Zoe, In spite of the awful things your partner went through, it sounds as some people somewhere in the NHS were listening in the end. Hang in there, they will hear you too eventually. They, or at least some parts of the NHS, have come through for quite a few us in the end. Just needs perseverance and patience---all not easy when in pain and penury.
