You have been dispatched to a nursing home today (Sunday) for a patient with altered mental status. Your patient, a female in her late 80s, has been describing vivid hallucinations since Friday, describing animals in her room. The patient has a mild case of early dementia which is well-managed by Aricept. The patient's history includes atrial fibrillation, a pacemaker, CABG, and a TIA. The patient is currently being treated at the skilled nursing facility for a case of pneumonia which was diagnosed on Monday. The patient is currently on Levaquin for the pneumonia. Her other meds include a beta-blocker, a diuretic, Coumadin, potassium, and Simvastatin. Vitals are all within normal limits and the patient is negative when you perform a prehospital stroke screening exam.
