I don't think the letter is a bad idea, even if it gets stuck in your chart. And although it is long, I honestly think that there is a 50-50 chance that the doctor will read the whole thing just out of curiosity (I probably would). The tone is respectful and no threats are made. However if you feel you must also make a complaint it would be better to file an official complaint rather than copy others on the letter for legal purposes.
