" The patient has Gilbert's syndrome, a hereditary condition that causes a reduction in the enzyme that conjugates bilirubin, resulting in increased bilirubin in the bloodstream. It occurs in 5-10% of the population and is a benign condition. However, when a person is under stress, dehydrated, as this one is, and so forth, the person may become jaundiced, which has happened here due to the patient's dehydration from prolonged vomiting and diarrhea. The cause of the patient's nausea/vomiting/diarrhea is salmonella that he got from eating the potato salad at the church social. He gets some IV fluid and is discharged to recover at home, which he does. The point is that not everything is always as it seems. All the guesses about liver disease were appropriate, but in this case, there's a different cause. It's true that history is very important, even when things seem clear. Some studies have found decreased risk for heart disease in patients with GS. It is thought that bilirubin acts as an antioxident. Google Gilbert's and you'll find some interesting stuff.
