This is a specific smell that results from liver disease. Usually a partner will notice first and a good Hepatologist/GI will ask the partner/spouse if they've noticed a bad or strange breath odour from the afflicted person. I'd ask my doctor if he/she thinks that is the cause. Prednisone can cause a range psychological/psychiatric problems including insomnia, depression, emotional instability. But so can chronic disease and chronic liver disease. Insomnia is a symptom of chronic liver disease and transplant may not cure it.
