There is an example and a template to copy and paste and fill in at

the bottom of this page:

It helps to give as much history and symptoms as possible.

If you have the test in a computer file from DDI, or scanned, you

could email it to DeanNetwork (deansta22 in the members section, or

click on his email in one of his posts). He has a website to post

results. The scanned results are nicer to look at.
