Dear All,

The Darrell Gwynn Foundation (DGF) is currently seeking individuals that are in

need of a new wheelchair. DGF is a non-profit organization located in South

Florida whose largest program is its national wheelchair donation program. In a

given year, we donate approximately 50 brand new, customized power and manual

wheelchairs to those that need them most. Below is the list of basic criteria

that one must meet in order to qualify for our program. If qualification is

determined, then there will be a formal application which must be filled out.

•Applicant must suffer or have suffered from an injury or illness that leaves

the applicant permanently confined to a wheelchair

•Applicant must be living with injury or illness for at least 6 months

•Applicant must be a US citizen or permanent resident of the US for at least six

months

•Applicant must be between 2 and 45 years of age

•Applicant must have an active role within the community or be able to

demonstrate the ability and desire to re-enter the community

•If under the age of 18, application must be completed and signed by a parent or

legal guardian

•Applicant must be able to provide documentation from a licensed physician

stating the medical diagnosis and permanency of the applicant's disability

•Applicant, parent or legal guardian must demonstrate financial need

•If applicant is covered under private insurance, a letter from insurance

company stating the reason(s) for denial must be submitted with application

Thank you so much for your response! The Darrell Gwynn Foundation actively works

with spinal cord injury support groups as part of our mission to provide support

for people with paralysis and prevent spinal cord injuries. We offer a Spinal

Cord Injury Education & Prevention Program that is presented in many high

schools throughout Broward and Miami-Dade counties. The program utilizes a

moving video presentation along with a speaker who has suffered a spinal cord

injury to warn high school students about the dangers of engaging in risky

behavior. is actually a speaker for our program and has a long history with

the foundation. We think he's the best!
