I have been asked to update a few hospital names due to the

incorporation of the " parent " name into the original name.

The hospital names in the attached document have been updated in our

system. I have included the Entity ID numbers along with the former and

" new " names.

This information should only affect you if you are looking for the

hospital by name in our system. All Entity ID numbers have stayed the

same.
