Hi Jeff,

I am also interested in the answers to these questions. I have pain

attack now every two/three weeks which lasts for about 1-4 hours

usually. The pain feels actually located more below the sternum than

URQ, although there is a spot on the right side below the ribcage that

does hurt when pushing on it. My hepatologist thinks that it is IBS,

because they never saw any stones or sludge on an ultrasound, the main

bile duct seems to be ok, and because he claimed that any intrahepatic

blockage should not hurt because of the lack of nerves (but wouldn't

the stress on capsule hurt?). Anyway, I am now wondering (hoping) that

my pain attacks are indeed not liver-related, but something like a

peptic ulcer, or IBS.

Can anyone describe the pain that results from blockage, is it constant

or with attacks, is it triggered by something, does it slowly go away

again (if it goes away) or more of a sudden as if the pain flushes

away, does it hurt without touching, does warmth (e.g. from a shower)

help cope with the pain, are there always other signs that show it was

liver related (dark urine, light stool, etc.)?

I have only recently starting experiencing this but, like you, mine starts in

the sternum area but goes under the ribs and wraps around under my right

shoulder blade. I have tried heat and while it helps, it does not get rid of

the pain. It has only happened a few times but there have been two times within

a month. I plan on talking to my GI about it on Thursday. I will the group

know what he says about it.

I have found my pain comes on rather quick, within 30

minutes I can be in such excruciating pain. It starts

right under my left ribcage and will radiate to the

back under my left shoulder blade. It gets more and

more intense to the point that I can't talk and will

start vomiting. The only thing that helps at this

point is a trip to the ER to get Morphine shot,

Benadryl to counteract the itching from morphine and a

shot to stop the vomiting.....then after about 4 hours

in the ER, when the pain meds wear off....usually the

pain, blockage has resolved it's self. Only once did

it not and required another trip to the ER the same

night.

I have been having pain in the upper right quadrant

[only recently radiating into my rt shoulder] all the

time but variable in the amount. First mild [couple of

years] then more mild into moderate, in about the last

three years. And sometimes accompanied by chills/low

grade fever for a day or two and

nuasea[blockages/infections]. Then the cycle started

to change. For some time now, may be within the last

4~5mo., the pain has been elevated into moderate to

high. But lately, even more intense or excruciating as

you have called it, specially afer meals, although i

am on a low fat or lately even a more light diet. But

the scary thing is that it is constant. And i did have

to seek help and so to run to the ER, too of course.

My GI doctor thought a part of it might be due to the

gall bladder. I did have a HIDA scan last July and was

told by a surgeon that the gb filling is about 43%

[35% is the min] which for my surprise meant a surgery

was'nt that necessary. But when the pain started to

intensify i was sent for another HIDA scan actually

yesterday, which did show that the gall bldder

contraction has even improved since the last test! I

am still waiting for the surgeon's opinion, i feel

kind of dissappointed, wishing or hoping [as Gerard

mentioned] that the pain would be due to it, at least

partially and not completely due to the bile duct but

that is what the scan, showed.

I am so anxious and stressed out for that. I was

thinking to have the gall bladder removed and may be

the pain would improves. But now i am not too sure. I

will be having an ERCP next week and see.

So i am wondering if you or any one in the group have

had a constant and a high URQ pain as what i have been

experiencing?

, This is exactly what I exprienced before. but as it may be an

inflammation there in your bile ducts so ERCP wouldn't be useful as I

exprienced it. they tried ERCP on me but no result they just found it

out I need an operation due to my advanced symptoms (jaundice,

itching, pain, indygestion, ...) so make sure what is their exact

intention in suggesting you ERCP? there must be a reason ! ERCP is

not 100% safe and its not that easy. so please don't go for that

unless you make sure why and even after other medical tests like

Ultrasound, CT Scan.

and dont worry about it most of us has gone through this and nothing

happened.

let me know what you are going to do.

Actually, i did have an ERCP appointment set for last

February, actually but i did postponed it and for few

more months, i thought. But because of this constant

very intense pain and thinking about a blockage, i

decided to do it sooner, in March. The Doctor which

usually perform it [out of my hospital] use to be my

UC doctor for some time and i do try to respect what

he suggest, but not a too soon, ERCP's. His way of

management of PSC is to do periodical ERCP's [3/6mo].

That is too soon, unless really needed as you said,

and i do dread them plus they are not 100% safe, as

you have mentioned, too. It has been about 10 mo since

the last one. I know it is still not that long, but

because of the change of the pain scale from

mild/moderate into a high/very high, continously and

without a break, i did decide to go with it.

I just had an Ultra Sound recently and a CT not too

long ago. And actually they are looking at the gall

bladder scan now to see if part of the pain is from

that since the pain tend to increase after meals and

usually with a lot of gas and irritation, produced.

Did you have a constant pain, too?

I will keep you posted, of course.

PSC is tough, though its test are more.

actually in my case the pain didn't start very soon, pain was started

after I got jaundiced and itching and fatigue. then URQ pain and how

easy I trusted to my doctor for an ERCP rightafter I did that

unsuccessful try I found I'm under very special care by those doctors

as they were shocked if something would happen while ERCP to me when

those ducts were like that.

I was going to die if he had made a simple mistake.

anyhow, what does your doctor expect to do in this ERCP ? and I'm not

sure if ERCP helps you to get rid of this pain because I barely hear

from psc patients with successful ERCPs like just right now hearing

from Cris. in fact I prefer to cope with pain than going into the

wrong way. so write a new post with this subject 'ERCP and PSC' and

ask the group to see how many of them have had successful ones. Cris'

daughter and I are regreftul about it.

Usually DRs do ERCP to make balloon in blocked ducts and for some

patients like me it's not more suggested due to my operations.

How did you make sure it's a PSC? could you find inflammation report

in your Ultrasound and CT ?

and what are your symptoms now?

My pain was such a very close friend to me after my meals I had to

cope by asking 3 persons to massage there at the same time.

it's very very very important to know the expectations of what your

going through.

Here ultrasound helps your more to know more about your condition.

UltraSound Report should show that where is inflammed and how much is

this inflammation and how is the flowing of your bile from gall

bladder to intestine. if they find out it's just in gall bladder

Laparoscopy is prefered than ERCP. but if its more adavanced in your

bile ducts so it's gonna be a deal to exprience an operation . and

now that these all may happen after each other one by one ! so why

not to stay brave and do the most right one. I mean I dont want you

to do everything I suggest you to find the best one.

Maybe your doctor got this concept to see how your condition is by

ERCP. is that the best? Im not sure as we check the other patients'

ERCP report. what you should know is that I was asked to go for

operation with no neglect.

I'm against ERCP. I don't believe in it for PSC patients.

for sure there must other tests to show how advanced your

inflammation and cholestasis are.

you should be very consious by checking everyone's ERCP report.

I'm counting on ultrasound in your case . do you have it scanned ?

if yes please send it to my email.

what is the result of your blood test in ALKALINE PHOSPHATASE, CA 19-

9, CEA, SGPT, SGOT ? the first two ones are more important.
