Guest guest
Thank you, Genevieve. I've been researching the net and it seems that both MRCP and ERCP are similar in their accuracy but MRCP "cannot really exclude early PSC". One wonders what the frequency of such "false negatives' could be... and how much time is needed before MRCP is able to detect it. Gastrointest Endosc. 2006 Aug;64(2):219-23 Diagnosis of primary sclerosing cholangitis: a blinded comparative study using magnetic resonance cholangiography and endoscopic retrograde cholangiography.CONCLUSIONS: ERC and MRC were comparable for diagnosing PSC, with very good interobserver agreement for the diagnosis of PSC and IHD strictures. Only ERC had good agreement for EHD strictures. Interobserver agreement was very poor for both MRC and ERC when disease severity of PSC was assessed. Clin Gastroenterol Hepatol. 2006 Apr;4(4):514-20. Diagnostic accuracy of magnetic resonance and endoscopic retrograde cholangiography in primary sclerosing cholangitis. CONCLUSIONS: PSC can be diagnosed with high accuracy and good interobserver agreement. MRC and ERC performed equally well in the diagnosis of PSC when used blinded to clinical information. Radiology. 2000 Apr;215(1):71-80. Primary sclerosing cholangitis: evaluation with MR cholangiography-a case-control study. CONCLUSION: MR cholangiography enables accurate detection and localization of PSC. Clin Ter. 2006 Sep-Oct;157(5):425-9. [Role of MR cholangiopancreatography in the evaluation of biliary disease] CONCLUSIONS: In our experience MRCP proved to be highly accurate as fundamental diagnostic step in patients with clinical and laboratory findings suggestive of biliary disease. The workload of ERCP, invasive method with risk of complications, in the diagnosis stage could therefore be reduced and its use be reserved for therapeutic indications.
