I can better understand what I'm reading and will ask if there is anything else that I can not understand

This is a good question. This practice was in place already when I joined, so I don't know the history of it. It is a sort of "short hand" to give pertinent information – such as

what is your relationship to PSC? You will see some with "Mom of …" or "husband of …"

when was the person with PSC diagnosed -- 1/04, PSC

Are there other relevant diagnoses or procedures– 1/04, UC (ulcerative colitis) ---- j-pouch (procedure done as a result of removing a colon)

Are you (or the person with PSC) on a transplant list – and where – Listed 7/21 (this year) at Baylor Dallas transplant center

Have you had a transplant – when – where ---- tx 1/04 UPMC Pgh

How many transplants have you had ---- tx1 tx2

Those are the short hands I can think of off the top of my head … keep asking! This was a good question.

I understand some of it:

- Son Ken (33) -

her Son Ken is 33 years old

- UC 91 - he was diagnosed with Ulcerative Colitis

in 1991- PSC 99 - he was diagnosed with Primary Sclerosing Cholangitis in 1999

but what does " Listed 7/21 @ Baylor Dallas " mean??

Not (this year) as

July hasn’t come yet, I’ll fix that date now. Thanks.
