Just out of curiosity, why did you remove them from protocol?

We are extremely fortunate in that respect. That's about the only thing

we're fortunate in, though. Arizona EMS is a joke. We have state mandated

protocols that are hopelessly archaic and restrictive. It takes an act of

the legislature almost to get anything changed. For example, in the state

of Arizona, paramedics cannot give fentanyl. Period. Only NURSES can.

We have no critical care paramedics here. They have " Critical Care Nurse

Units " that do the critical care transports, or else they send them by helo.

They LOVE helos here.

Few services have capnography, even fewer CPAP. Chest pain protocols make

no mention of right sided MIs, and MONA is prescribed for ALL chest pain

with a BP of >90. We all know what can happen if you give a slug of NTG to a

patient with RVI and a BP of 90. Sigh. None of the services here have

their medics do a right sided ECG; the medics don't know what you're talking

about.

Folks, don't EVER EVEN THINK about allowing Texas to go to a system of

state mandated protocols and scope of practice.

We changed over to the King Airway, mostly out of physician and medic

preference. Some had some difficulty getting good seals, especially on larger

patients. These were probably difficult to start with, however.

I don’t think there is anything wrong with them. The O.R. uses them

extensively.
