Question for incoming 6th graders

Hi,

My daughter is starting middle school next year, going into 6th grade. Does

anyone know if she will need new blue/yellow medical forms? I think I heard

somewhere that whenever your child starts a new school, new forms are required.

The schools always called me for new ones however I told them to get it from the previous school. Once the blue medical exemption form is signed it is for life and follows the child to whatever school he/she goes to.
