This is a horrible disease both physically and mentally. As for how it starts, there is no one answer. For some, it starts from some traumatic experience, whether phical or mental, ie: a car wreck, or someone dying. Sometimes, it just comes on gradually with no way to pinpoint any trigger. From what your wrote, it sure sounds like you have all the signs and symptoms of Fibro, unfortunately. I hope you get a diagnosis and then get treatment for the pain and anguish. My family tries to understand but like the topic on friends understanding my husband can be unsupportive and quite cruel at time. I guess I just need to talk to others that do understand.
