The trick for us was to switch from a Bivona to a Shiley trach which had less of a " v " shape. He has been wearing the Shiley since then without any problems. Guest guest Posted June 7, 2007 Report Share Posted June 7, 2007 Hello, My son Jonas, who is 3 years old-CHARGE, has a 3.5 Bivona, and we use a 2x2 split gauze underneath the flanges. I met a mother at a doctors appointment in Philadelphia, and she was showing me her sons sores, I told her to use the gauze, and the trach collar was way to tight, you should be able to fit 2 fingers underneath the ties. I've attached a picture of Jonas's trach, if you want to look at the gauze we use. Good Luck.
