I just got more recent lab work done and it looks like my RT3 ratio is 5.95 (if

calculating correctly), back in January it was .814. I'm not sure what this

means and would love some guidance. I've been trying to work my way up on t3 for

the past several months and I am up to 20 mcg. Along with that I'm taking

isocort 5 in am, 5 at noon, 2.5 at 4pm, and 2.5 at 8 pm. I have been exhausted

since increasing from 15 to 20 mcg of cytomel. i also felt extremely exhausted

the last time I increased as my dosage as well. Is this normal to feel so

fatigued...I'm trying to figure out what else could be contributing to this or

if I could be causing more damage to myself by doing this.

My recent labs are as follows:

(the older lab results are in the previous email further down the page)

Triiodothyronine (t3) 106 ng/dL range 71-180

Reverse T3 178 pg/mL range 90-350

TSH 3.85 uIU/mL range .45-4.5

Thyroxine (t4) free

Direct, S t4, Free (Direct) .78 ng/dL range .83-1.77

Ferritin 47 ng/mL range 13-150

My adrenals were tested last December '09. These are the results. Should I be

getting these re-tested?

Optimal Range ng/mL

5.3 Morning: 7-10 ng.mL

2.8 Midday: 3.0-6.0 ng/mL

2.6 Evening: 2.0-4.0 ng/mL

2.6 Night: <1.5 ng/mL

Any thoughts on what is going on would be so greatly appreciated. Thanks so

much.

Reverse T3 178 pg/mL range 90-350

You have no Free Triiodothyronine test so we have to use the TOTAL T3 so we are

lookin for 10 or higher for a ratio.

Your ratio is 1060/178= 5.95 WAY too low.

I have been feeling extremely tired and it seems to correlate with raising my

dose from 17.5 to 20 mcg of cytomel. Is this a symptom that is normal and my

body will adjust or that I can't go any higher at this point because this is

pointing to other issues like adrenal or ferritin. I get to a point several

times in the day where I just have to lie down...my body just feels so exhausted

and limbs feel limp and heavy.

I'm dosing like this:

4 isocort and 2.5 mcg at 8am

4 isocort and 2,5 mcg at noon

2 isocort and 2.5 mcg at 4 pm

1 isocort and 2.5 mcg at 8 pm

You NEED to get iron albs as this sure sounds liek low iron. But you

can;tjust assume thta Iron panels are CHEAP! You can order one for

yourself thoruhg the links in our LINKS section for labs. I htink

www.econolabs.com has them for like $23 so really, get tested.

OK your iron labs do not look low so I htinnk we shoudl assume it si

adrenal stress you are feelign. I woudl think it is time to try some HC.

I just read something that said my ferritin should be at 50 to start t3, but

ideally it would be between 70-90. Mine is at 47, do the other test results on

the iron panel indicate that my iron results are ok?

In terms of the HC, how should I start dosing? Is it possible to try HC cream

first and what dosage would you recommend and how many times a day?

I'm hesitant to go on HC, would higher doses of isocort work? Right now I'm

doing 5-5-2-1 of isocort.

Yur serum iron is prety good and your % saturation is actually almost

high so I would not recommend taking iron wiht theze labs. You binding

capacity is low as well which indicates your body canntpo tolerate more

iron.

Yes just look for 1% Hydrocortisone cream with the least amoutn of

objectionable bases, like no petrolatum. You want a ream not a gel.

You would ue it just liek you would use HC pills. IN a pattern

through the day from higher in the AM to lower in the afternoon.

Thsi needs ot go to the adrenals group as it is really not an RT3

topic.

Hi Val,

You mentioned that I should stop taking iron based on these lab results.

Originally my ferritin serum was at 20. If I stop taking the iron will it go

back down? Should I keep taking a moderate amt?

Also I'm a bit hesitant about taking hc. If I were to try that or the cream what

is a good dosing place to start? Do I work my way up or start with the amount

believed to be needed for my situation.

Should I stop iron altogether? Originally my serum level was 26 and now it's 47.

I'm afraid if I stop completely it will go back down. What could be the reason

for not being able to tolerate more iron? Adrenal fatigue?

Long term illness reduces the binding capacity. Also LOW T3. As you

raise T3 you should be able to hold iron better and htis si one of the

reasons why, Binding capacity goes up and so does other things such as

RBC that help your body hold onto iron.

I'm wondering if I should stop taking iron altogether based on the % saturation

being high. Originally my iron was at 18 and I've gotten it up to 47 and I'm

concerned if I stop taking it that it will go back down.

I'm at a loss for what to do. If I can't get my iron up then I can't handle more

t3 and then my adrenals are stressed. How would you recommend working on this?

You simply need ot raise the iron slower than someone wiht high

binding capacity. 50mg a day for someone liek you will eventually

increase the nbinding capacity and build the iron and pushing T3 as much

It wil take someone wiht lkow binding capcity longer to clear Rt3 an got

to a proper dose of T3 than someone wiht nor,mal iron levels or good

binding capacity but it can stil happen, just takles a bit loinger and

more patience.
