As you raise the T3 and the tissue begin to get good levels, your

SHBG can rise and bind some of that T3as well as estrogen dn

testoisterone. This jutst happened ot me and I had ot raise th dose of

T3 to compensate, But thne I raised estrogne and testosterone too so

really went hypo as all three raise SHBG.

regarding this about shbg rising when T3 rises..in a young person with ovaries does the person adapt to that increased shbg on their own??

Yes if ovarian production is normal they wil pick up the slack

easily.

When we get our period and the estrogen drops, does that leave more SHBG to bind

t3? I just got my period and went SO hypo and I have no idea why because I don't

recall going really hypo usually when I get my period.

It's either that or t3 not getting to cells because I have been trying not to

stress dose although I felt very low cortisol symptoms yesterday, felt like I

was going to collapse, actually, was very out of it and spaced out and sort of

vibrating.

Actually SHBG is HIGHER when Estrogen is high as is Thyroid Binding Globulin.

SHBG & TBG will be lower when Estrogen is low. Of course Estrogen is not the

only thing that affects SHBG. T3 levels, DHEA, Testosterone all have an impact

as well.

Progesterone also drops when you have a period. That can impact Aldosterone,

Sodium, Potassium & even Cortisol levels too. If Progesterone is too low it

certainly impacts Thyroid indirectly if it impacts Cortisol levels. Electrolyte

imbalances also cause Adrenal stress as more cortisol/Adrenaline can be released

then.

What are your Sodium, Potassium levels like? Have you tested Aldosterone at all?

Tried taking extra Salt in warm water to see if it helps?

Sounds like you should have stress dosed yesterday. You may be paying now for

not doing it when needed. Try a small stress dose 5mg HC see if it does help?

Thanks,Lethal. Okay, so the SHBG levels drop as your estrogen drops. Ok.

Yes, I know PG drops signficantly during your period and that's what actually

brings on the menses, but since it doesn't bind SHBG, I didn't mention it. Just

didn't want you to think I'm a total idiot. :-)

My PG is not low as I supplement to keep it at the higher end of the luteal

phase, but of course it goes low (as it should) when I get my period. But as

I've said, I've never gone hypo before during my period, so I think now it was

maybe just a coincidence.

As for the aldosterone, etc. going lower as pg goes lower, actually, I am on

..125 Florinef for a couple of months now. Helped me immensely as I had almost

zero aldosterone. The last time I checked a few weeks ago both sodium and

potassium were excellent, but I mean to check again next week.(I'm out of town

right now, hence the stress!) Extra salt didn't help, just makes me swell up

now, so I think I have plenty of sodium and I hang onto it fine because of the

Florinef.

Yes, I agree it was more likely the lack of stress dosing. The reason I didn't

stress dose is because I have just reduced down to 37.5 HC. (I got onto too high

a dose when I wasn't on the Florinef.) So Val thought I should try not to stress

dose unless absolutely necessary (like an accident or something) as that's a

pretty high dose. However, I think the fact is that my BODY is used to that

higher dose (and I'd only been on that reduced dose about 10 days) and so when I

got stressed from a very long day of travel, my body wanted some more cortisol.

I think I should just have stressed dose and then gone back to the 37.5 when I

get home and continue reducing slowly. That was my gut feeling anyway. I don't

think 50 HC for a few days on a trip can kill you and may be what our bodie

would make anyway on a very long, tiring trip.

Hi Kathleen,

When I was weaning down on my Adrenal meds (Prednisolone daily + HC for stress

dosing) I was only able to do it by stress dosing frequently. I needed to do it

a LOT more often than when I was on a higher dose of Pred.

When I stress dosed I did it with small doses only 4 mg HC at a time as I needed

to. And I needed it to do anything much like do the shopping, or go to an

appointment, etc. I would repeat the 4mg Hc dose IF the first wasn't enough

after 45mins or so.

I weaned very slowly only 1/2mg Pred (2mg HC equiv) at a time & only reduced by

2mg HC a MONTH. I did get down from 40mg HC equiv (was on for almost 2 years) to

my current 28mg HC equiv. Been on that now for 4 months & rarely need to stress

dose at all anymore.

In fact now thinking of weaning down some more ... might see if I can get down

to 20mg HC Equiv (5mg Pred).

Yes, I have reduced pretty fast, from 45 to 37.5, so i'm sure my body is BARELY

used to the 37.5 on regular days, let alone stressful ones. Sigh. But I was on

way too much, so I had to get down.

I want to make sure I understand correctly. So you took stress doses while

weaning. But how could you wean then? If we are stress dosing all the time, how

does the body get used to the lower dose we are trying to get to? So you must

mean you would stress on days you had to (like to go grocery shopping) but

certainly not every day, correct?

So on the days you DID stress dose, did you end up taking as much steroid as you

were previously on? I mean, if your original dose was equivalent 40 HC and your

new goal was about 37.5 HC, on stress days, how much did you take and then did

you just go back to your " new " dose of 37.5 the next day?

I am so confused about stress dosing while trying to reduce in general. I know

Val told me that while trying to reduce, you need to try to keep stress to a

minimum so you don't have to stress dose and your body can get used to the new,

lower dose. But that was impossible this week. :-(

Do you think that by not stress dosing, it has made me feel hypo, i.e., the t3

not getting to the cells due to lack of enough HC? Or doesn't it work that way?

Hi Again,

I stress dose only WHEN I need to. I only do SMALL doses 4mg HC. And only

repeated the dose IF still needed more say 45-60mins later. No I don't 'wean' a

stress dose. Just go back to 'normal' daily dose immediately.

If I was having a MAJOR stressor say an infection I may double my normal dose.

If I needed to do that for more than 2-3 days THEN I would wean down from that.

You wouldn't stress dose 'all the time'. But when weaning/titrating down it does

help the process if you DO stress dose as/when required.

That enabled me to keep weaning gradually. If I hadn't done that I would still

be on the higher dose. So yes I do mean I only stress dosed IF going shopping,

to an appointment, etc stressed me.

Anyway that worked for me. As time went on the need for MINOR stress dosing

reduced. Now I'm only needing to stress dose for MAJOR stressors again.

I fully expect that if I try to titrate down further I will again need to do

'minor' stress dosing again at least for a while.

Thank you so much for explaining that. I've always been awful about not stress

dosing and thus get into trouble and crash. Plus, for my current situation, I

was thinking that if I'm trying to reduce my daily dose, it " ruins " it by taking

a stress dose. Which I realize doesn't make sense as my body is not used to the

new dose yet and needs a bit extra for stress. So your explanation below on how

you personally handled it helps me a lot. Thank you!
