My school district is not going to like me very much after I give this to them and tell them to write the whole thing over on the new form, like they should have done in the first place. I'd call Barbara Gantwerk's office (director of Spec ED programs in NJ) but I can't find her # OR Randi Burton or Sharon Gottlieb Bergen Co Supervisors of the CST (201)599-6256. And like Cheryl said COSAC could help. If you still don't get anywhere with your district, time to get in touch with an advocate or lawyer.
