York State death certificates (on the second page of the certificate -- No

cause of death is listed on the first page for confidentiality) it has a)

immediate cause of death and under that line due to or as a consequence of:

In my husband's case his death was listed as: cardiopulmonary arrest due to

Creutzfeldt-Jakob Disease..................Pat
