Would you mind saying again exactly what information you need on the CJD

victim? I know you want name, age, date of death, occupation, and place of

residence. Is there anything else? I remember you asking for suggestions -

didn't notice if there were any. Thanks!

link there to SUBMIT your info. It is a form. Just fill out the form, high-

light it and copy it, and paste it into your email. If you have a problem

just print out a copy of the form and then write your email using the form as

a guide of what we need.
